When to use this debridement template

Use it when plaque and calculus are so heavy that the dentist can't complete a comprehensive evaluation. D4355 is a preliminary procedure. Its job is to make an evaluation possible at a later visit, and the CDT descriptor says it should not be done on the same day as D0150, D0160, or D0180. Payers often deny it when the note reads like a regular cleaning.

What makes a D4355 note hold up

Example D4355 note

Patient seen for full mouth debridement (D4355). Last professional cleaning more than 5 years ago. Findings: heavy generalized supragingival calculus and calculus bridging interproximal spaces, with severe generalized inflammation. Deposits prevented accurate probing depths and assessment of tooth surfaces for caries, so a comprehensive evaluation could not be completed today. Intraoral photos taken. Debridement performed with an ultrasonic scaler and hand scalers; full mouth completed. Tolerated well. Comprehensive evaluation scheduled in 2–4 weeks, with full-mouth periodontal charting.
